    Effective September 1, 2025

    Texas Health and Safety Code §181.105

    This notice provides instructions for consumers on how to request health care records, contact the appropriate licensing council, and file a consumer complaint.

    1. How to Request Your Health Care Records

    You have the right to request access to and copies of your health care records.

    To request your records from this licensee:

    A. Submit a written request for your health care records. Requests may be submitted by mail, secure electronic communication, or in person, consistent with our office procedures.

    B. Include the following information:

    Your full name

    Date of birth

    A description of the records requested

    Preferred delivery method (electronic or paper)

    Your signature and the date of the request

    C. Requests will be processed in accordance with applicable state and federal law, including timelines and allowable fees.

    2. How to Contact the Licensing Council

    You may contact the licensing council that oversees this licensee.

    The Council’s website provides contact information to submit questions, concerns, or requests for information by phone, email, or online form.

    3. How to File a Consumer Complaint

    If you wish to file a consumer complaint, you may do so with the Texas Office of the Attorney General (OAG)

    To file a complaint:

    1. Visit the Consumer Protection webpage linked above.

    2. Complete the online consumer complaint form or follow the instructions for submitting a complaint by mail.

    3. Provide all requested information and supporting documentation to assist in the review process.
